  • You will not be seen by PAs, NPs, or other mid‑level providers.

Because we provide comprehensive, individualized care, patient participation is crucial. Compliance ensures:

Patients are expected to follow treatment plans, attend scheduled follow‑ups, and complete recommended testing.

Patients are responsible for:

  • Completing all ordered labs, imaging, and diagnostic tests promptly
  • Scheduling a follow‑up appointment to review results
  • Ensuring results are received and discussed with the provider

Results cannot be reviewed by text, email, or messaging. A dedicated appointment is required. 

Patients receiving biologic treatments for:

  • Atopic dermatitis
  • Asthma
  • Chronic skin conditions
  • Nasal polyps
  • Eosinophilic esophagitis and other GI issues must maintain regular follow‑up appointments.

Biologics require ongoing monitoring for safety, effectiveness, and insurance authorization. Missed follow‑ups may interrupt treatment.

No. Medical care—including advice, diagnoses, treatment changes, and result reviews—is not provided via text or email.
Patients must schedule a dedicated appointment (in‑person or virtual) for all medical discussions.

Electronic communication is used only for administrative matters. 

Because we do not double‑book and each appointment is reserved exclusively for one patient:

  • 24‑hour notice is required to cancel or reschedule
  • late‑cancellation fee applies to changes made with less than 24 hours’ notice
  • no‑show fee applies to missed appointments

Yes, we accept a limited number of insurance plans.

Due to decreasing insurance reimbursements, reduced coverage, and administrative burdens that interfere with patient care, our accepted insurance panel is intentionally limited.

Good Faith Estimates

We provide a Good Faith Estimate for all services.

If your deductible is not met

  • The estimated cost of your visit and testing is collected up front.
  • After your insurance processes the claim, you may owe an additional amount based on your Explanation of Benefits (EOB).
  • If there is an overpayment, a refund will be issued.

Important note

The financial relationship between you and your insurance company is separate from the medical practice.
We assist with billing, but we do not control:

  • Deductibles
  • Copays
  • Coinsurance
  • Coverage decisions
  • Denials or delays

Out‑of‑network patients

For patients who are out of network:

  • We offer fair, transparent, up‑front pricing
  • We provide a detailed receipt (superbill) that you may submit to your insurance for possible reimbursement
  • Some plans reimburse patients directly for out‑of‑network benefits

Your provider reviews your information before your visit to ensure your appointment is focused entirely on care. Incomplete forms may require rescheduling.

Because we do not double‑book:

Missing follow‑ups or refusing essential testing limits our ability to provide safe care. Repeated non‑compliance may result in:

  • Delayed diagnosis

  • Possible dismissal from the practice

Yes. Patients may be dismissed for:

  • Repeated non‑compliance
  • Failure to complete forms or payments
  • Multiple missed or late‑cancelled appointments
  • Abusive or disrespectful behavior
  • Dishonesty or falsification of medical information
  • Misuse of services
  • Violation of any office policy

A written dismissal notice will be provided with instructions for transferring care.

Patients who arrive 15 minutes or more after their scheduled appointment time may be unable to be seen and the appointment may be considered a no-show. In such cases, the appointment may be forfeited, and a $75 no-show fee may be required in order to reschedule. This policy is intended to help us maintain timely care for all patients. 

If you are experiencing a medical emergency or a life-threatening situation, please call 911 immediately or go to the nearest emergency room. Our office is not equipped to handle emergencies.
